About us
Zumbro Valley Health Center is a private non-profit organization that provides mental health, substance use disorder, primary care, and community-based services to twelve counties throughout southeastern Minnesota. We believe that improving our clients’ mental health outcomes is not achieved by a single person or program, but rather it’s a person-centric, coordinated approach to their care. We are grateful for the opportunity to continue supporting individuals toward healthy and fulfilled lives for years to come. Our professional staff works together with clients and others in the community to develop recovery-oriented treatment plans around each client’s distinct goals. Our person-centric, coordinated care approach includes a range of services that focuses on improving our clients’ overall health. Using an integrated model of care, we share information among team members relating to client care and establish a comprehensive treatment plan to address the physical, psychological, and social needs of each individual client. Each health care team includes a diverse group of members such as physicians, nurses, psychologists, therapists and other health professionals – all focused on the needs of the client. Our goal is to equip people with the tools necessary to create the healthy, rewarding life that supports their personal values.
