About us
The UCSF Center for Vulnerable Populations (CVP) at Zuckerberg San Francisco General Hospital and Trauma Center is dedicated to improving health and reducing disparities through discovery, innovation, policy, advocacy, and community partnerships. Founded in 2006, the UCSF Center for Vulnerable Populations is based within the UCSF Department of Medicine, Division of General Internal Medicine located on the campus of Zuckerberg San Francisco General Hospital. Beyond the local communities it serves, CVP is nationally and internationally known for its research in health communication and health policy to reduce health disparities, with special expertise in the social determinants of health, including literacy, food policy, poverty, and minority status, with a focus on the clinical conditions of pre-diabetes, diabetes, and cardiovascular disease.

🏆 Bravo! Dr. Alica Fernandez & Dr. Rebecca Sudore have each individually earned the 2024 UCSF Faculty Senate Distinction in Mentoring Award! As UCSF’s biggest mentoring honor, this award recognizes Dr. Fernandez and Dr. Sudore’s towering commitments to and achievements in mentorship! Dr. Fernandez is Professor of Medicine and Associate Dean of Population Health and Health Equity for the UCSF School of Medicine. She’s founding Director of the UCSF Latinx Center of Excellence and directs CVP’s Latinx and Immigrant Health Research Program Dr. Fernandez is a member of the UCSF Academy of Medical Educators. and leads several workforce diversity programs at UCSF. Dr. Fernandez serves on the National Academy of Science Roundtable on Health Literacy, the Patient Centered Outcomes Research Institute Board of Governors, and the Board of Directors of the American Board of Internal Medicine. Professor of Medicine at the UCSF School of Medicine, Dr. Sudore is a geriatrician, palliative medicine physician, implementation scientist, and professor of medicine in the Division of Geriatrics. She directs the Innovation and Implementation Center on Aging & Palliative Care Research and the Vulnerable Aging Research Core of the National Institutes on Aging-funded Pepper Center. She is also creator & director of the evidenced based, online PREPARE for YOUR Care advance care planning program, the PREPARE easy-to-read advance directives (available for all 50 US states in several languages), and the online PREPARE for THEIR Care caregiver program. ...CVP is honored to work in partnership with Dr. Fernandez and Dr. Sudore—dedicated faculty and generous mentors assisting the next generation clinicians & researchers.
