Thank you, Eric...great post. Medicare will continue to pay disproportionately higher rates to hospital-owned ASC's which will blunt the effect of shrinking the inpatient only list. They have historically used price controls to direct patients to the politically more powerful hospitals.
#Medicare Changes Inpatient Only List-->#1 Reason for Shift to Outpatient Services-->Implications for Employer-Sponsored #HealthPlans. Medicare has a list of approximately 1,800 procedure codes for #procedures that it will ONLY pay for on an inpatient basis. These tend to be more #invasive surgeries such as a coronary artery bypass graft (CABG). However, starting in 2021 CMS starting #removing procedures from the Inpatient Only List, meaning these procedures can now be paid for on a outpatient basis at an Ambulatory Surgery Center (#ASC). Two of the most important surgeries to come OFF the Inpatient Only List are #TotalKnee and #TotalHip Replacements. As a result, much of #hospital revenue is moving from inpatient to outpatient. Major hospital systems are dramatically #growing their ASCs so that competing ASCs do not take their patient volume. Hospital System Owned ASCs still have very high reimbursement rates from #CommercialInsurance for patients on an #EmployerSponsored health plan. Just because the procedure is at an ASC, that DOES NOT mean it is #inexpensive. Rather for employer-sponsored insurance plans, it is to their advantage to have plan members have outpatient surgery performed at an #Independent ASC that is not owned by a major #HospitalSystem. For example, the NTTC ASC in the Dallas Area charges $20,000 as a bundled price for a total knee replacement vs. over $70,000 for the same procedure at a hospital-owned ASC. It is very common for people not to care about the TOTAL price of healthcare services, and only about their own out of pocket costs, like copays, and deductibles, etc.... This is another major mindset in healthcare that simply must change. Read a comment online this morning, "it makes no sense to cash pay when my deductible is met." Yes, I understand....once the deductible and OOPM is satisfied, people completely stop caring about the pricetag. There is a severe disconnect with many people's understanding of the total price of healthcare and the impact it has on people's premium levels and plan designs. Because overpaying for care is an indirect and delayed penalty, people disassociate why their premiums keep marching in a Northeasterly direction year over year. People must start caring about the TOTAL cost of care....not just what their out of pocket costs are.
