San Fernando Community Health Center (SFCHC) is a state-of-the-art Federally Qualified Health Center located in the City of San Fernando and serving the entire San Fernando Valley. We are a patient-centered medical home, where you will be provided with compassionate, high quality, and evidence-based care. The FQHC designation allows us to provide quality care at an affordable price to everyone in our community. SFCHC is located at the corner of Mott Street and Chatsworth Avenue, near public transportation and freeways with ample free parking available. Our mission is to provide high-quality state-of-the-art health care services, as well as prevention and education services in a supportive atmosphere to every person, particularly the most vulnerable of the San Fernando Valley, regardless of religion, race, age, sex, or personal income. Our vision is that San Fernando Community Health Center will meet the needs of every patient by offering quality health care that is patient-focused in delivery and maximizes all available resources regardless of income or ability to pay. SFCHC is dedicated to achieving excellence in healthcare for the community of the San Fernando Valley.   • 𝗖𝗢𝗠𝗠𝗨𝗡𝗜𝗧𝗬 𝗣𝗨𝗕𝗟𝗜𝗖 𝗛𝗘𝗔𝗟𝗧𝗛 𝗧��𝗔𝗠𝗦 (𝗖𝗣𝗛𝗧) We’re excited to be part of this great community resource. • On Saturday, February 24, the Community Public Health Teams (CPHT) project hosted its first community convening at the Boys & Girls Club of San Fernando Valley. The participating team was introduced and shared more information about how CPHT will serve communities in Pacoima and San Fernando. Special thank you to all who took time out of their day to attend, share their feedback, and spread the word. Your participation is very important to the CPHT project.   • The California Department of Health Care Services has awarded San Fernando Community Health Center (SFCHC) $382,000 to increase the number of patients receiving enhanced care management services (ECM) and help improve health outcomes for Medi-Cal members. As one of 28 community-based agencies in Los Angeles County to receive $144 million in collective funding, SFCHC will use its share to increase and train staff, enhance infrastructure and information technology, and deliver intensive case management services. Care Health Plan, an organization dedicated to providing the most vulnerable residents and communities of Los Angeles County access to quality health coverage. The L.A. Care Health Plan has selected SFCHC as one of 15 community health facilities to receive a $100,000 grant to help increase access to dental care in underserved communities. SFCHC will use the funding to provide kindergarten students with important oral health screenings, as well as to educate them on the importance of oral hygiene and good nutrition. A key goal is to establish a relationship of trust among the children and the dental professionals who care for them. “At San Fernando Community Health Center, our model of care holds the wellbeing of the whole person at its center,” said SFCHC Chief Executive Officer Audrey Simons. “A growing body of research shows a direct link between poor oral health and many chronic health conditions, such as heart disease, stroke and diabetes. This funding will help ensure children receive dental screenings, begin a lifetime of oral healthcare management and find a dental home at an early age. “ The L.A. Care Health Plan also has awarded SFCHC a Robert E. Tranquada Safety Net Initiative grant in the amount of $175,000 to provide persons dealing with health inequities access to weight management programs, as well as screenings for chronic kidney disease, high blood pressure and other chronic diseases. The funding will support the recruitment, hire and retention of culturally congruent and diverse medical staff. “The funding will allow SFCHC to hire a registered nurse and a certified community health worker to further strengthen our primary care team,” said Simons. “The focus of our project is to increase the percentage of patients achieving blood pressure control, increase screening for chronic kidney disease and reduce obesity among patients, particularly those with underlying medical conditions.   • 𝗦𝗮𝗻 𝗙𝗲𝗿𝗻𝗮𝗻𝗱𝗼 𝗖𝗼𝗺𝗺𝘂𝗻𝗶𝘁𝘆 𝗛𝗲𝗮𝗹𝘁𝗵 𝗖𝗲𝗻𝘁𝗲𝗿 𝗥𝗲𝗰𝗲𝗶𝘃𝗲𝘀 $𝟮 𝗠𝗶𝗹𝗹𝗶𝗼𝗻 𝘁𝗼 𝗢𝗽𝗲𝗻 𝗦𝗮𝘁𝗲𝗹𝗹𝗶𝘁𝗲 𝗖𝗹𝗶𝗻𝗶𝗰 -- Thanks to the vision and leadership of Assemblywoman Luz Rivas (D-San Fernando Valley), San Fernando Community Health Center (SFCHC) has been awarded $2 million in state funding to open a satellite healthcare clinic in Panorama City serving those most in need. The satellite clinic will be located within the footprint of the Corazón del Valle housing project that is currently under development by Holos Communities, a non-profit organization dedicated to creating safe and equitable living spaces. Together, SFCHC and Holos Communities are working to ensure housing and high-quality medical, dental and behavioral healthcare are delivered to community members and low-income residents of Corazón del Valle. In addition, the globally renowned firm Perkins&Will is generously providing pro bono architectural and design services. SFCHC is also grateful to Thornton Tomasetti and tk1sc for contributing their engineering expertise, and to The Valley Economic Alliance and Goodwill Southern California for cultivating community and industry support. Through the collaboration of our project partners, this beautifully designed housing development will help increase health equity, assist the unhoused and strengthen our community. “Corazón del Valle is a direct result of community leaders working in partnership to build a healthier, sustainable environment for the most vulnerable and underserved,” said SFCHC Chief Executive Officer Audrey Simons. “When our satellite clinic opens in the Summer of 2024, it will be possible for our providers to deliver care where people live, work and play.
