Maria was evaluated for wound care in December 2023. She had been admitted to the center at the end of last August with acute hypoxic respiratory failure, cardiogenic pulmonary edema, and CKD, requiring dialysis. Maria had a new onset of AFib and had a pacemaker placed. She is diabetic, hypertensive, and obese. Unfortunately, she also suffered a left tibial plateau fracture during a transfer and was deemed non-weight-bearing on her left lower extremity. Due to her prolonged non-weight-bearing status and inability to get out of bed frequently, Maria developed an unstageable pressure injury on her sacrum. By December 2023, it was clear that her wound required specialized care. Our PTs were called in to initiate Closed Pulsed Irrigation (CPI) treatment to her sacral wound. With Maria‘s complex medical history, we weren’t sure how quickly we would see improvement, but we were determined to make a difference. We scheduled CPI treatments five days a week and saw remarkable results within two months! Maria actively participated in transfer training, offloading, positioning interventions, and received education on wound healing. Our PT and OT teams worked collaboratively with nursing and Maria’s nutritionist to ensure optimal wound treatments, dressings, hydration, and nutrition. Today, Maria’s wound is completely closed, reducing her risk of infection and significantly improving her mobility. #bettercarestartswithyou
