December started with an incredible group convened by NODE.Health's Digital Medicine Conference. Grateful Physician Innovation Network could participate in my own back yard this time :)

So, a few things to remind everybody. First of all, don’t forget EHRs (electronic health records) were purpose built originally for billing. This is no secret. People quite openly have called EHR systems glorified cash registers. If I want to be generous, maybe I would restate this to say that EHRs were designed to document patient interactions. This is what their core architecture was built to achieve. But today, there’s a lot that goes on that isn’t a traditional patient interaction. Me even calling it, frankly, a patient interaction should give longtime listeners a clue where this is headed. I mean, say you’re sitting at home on your couch. I don’t know. You’re probably not considering yourself a patient. You’re considering yourself a person sitting on your couch. However, say you’re sitting on your couch and you haven’t taken your COPD maintenance therapy. Potentially that is something of clinical significance that maybe should get figured out and noted somewhere—potentially prior to the acute event going down. PART 2 - NODE.Health Conference #DMC23 Dr. Geeta Nayyar, MD, MBA talked about the pervasiveness of disinformation and misinformation in healthcare. Delivering evidence based messaging builds trust, ultimately improving patient engagement and experience. Defining metrics measuring trust and loyalty are essential. What gets measured, gets improved. Kenneth Mandl mentioned physician adoption of AI if it’s useful and helpful in their day to day. GPT has been cited as being more empathic than its human counterparts. Thomas Fuchs heads the AI department at Mount Sinai. He talked about the importance of FDA regulation if AI systems are clinically impacting patients. These systems must be tested rigorously to ensure they are safe, effective, and equitable. The FDA has yet to approve continuous learning AI due to risk of drift. Every model cleared today is solely deterministic. Chris Gibbons MD, MPH shared how crucial it is for us to mindful of what we are feeding - and how we are training - the AI models. If you can’t explain it, you value the model for its outputs. That is biased. Transparency and explainability matter. I had a conversation with Grace Cordovano, PhD, BCPA over lunch. She has to be one of the most passionate and knowledgeable patient advocates I have ever met! Thank you to Ritesh Patel for facilitating the connection. Federal Health Innovation day on Friday was engaging and informative. Big shoutout to Stephen Konya for organizing the programming. This event helped to put faces to names across the various government entities while allowing us to learn about how to get involved. Micky Tripathi of ONC spoke about his long term goal of creating health equity by design. Could transparency of AI tools in the system be presented in a summary model card like a nutrition label on food? Key questions to consider for the label card: What was the tool designed to do? Where was it trained? How long ago was it trained? What were characteristics of the population it was trained on? How is the algorithm maintained and updated? The goal is for more responsible and equitable use of AI while maintaining respect for trade secrets & IP.
