About us

We are the Midlands largest homeless center providing homeless men and women ages 18 and older access to our day center, hot meals, showers, service providers and housing. Transitions is operated by Midlands Housing Alliance.

  • Happy Feel Good Friday! Meet Berlin, a shining example of perseverance and success! Berlin joined Transitions through our Outreach program in February 2023. Throughout his journey, he has worked with numerous dedicated case managers and moved up to our Men's Program. With the help of our partnering agencies, Berlin secured affordable senior housing complete with all amenities. During his time at Transitions, he volunteered his time doing community service and stayed committed to maintaining his sobriety, thanks to connections made by his case manager. We are incredibly proud of you, Berlin!

  • Happy Feel Good Friday! We'd like to share Nola's journey to permanent housing. She arrived at Transitions last November, facing substance abuse and without family support. Through our Actively Aging program, she discovered the joy of community living. Nola followed the program diligently, gained acceptance into a community residential care facility, and has successfully moved out. She has stayed sober and even reconnected with her granddaughter. We're so proud of Nola's incredible transformation.

  • Juneteenth, also known as Freedom Day or Emancipation Day, commemorates the emancipation of enslaved African Americans in the United States. On June 19, 1865, Union soldiers arrived in Galveston, Texas, with news that the Civil War had ended and that enslaved individuals were now free. This momentous occasion marked a critical milestone in our nation's history, symbolizing freedom and liberation for so many. At Transitions, we recognize the importance of honoring Juneteenth as a day of reflection, celebration, and learning. This year, we encourage everyone to take time to educate themselves about the significance of Juneteenth and its impact on American history and culture.

Since opening our doors in 2011, we've proudly placed 3,673 individuals into permanent housing, served 29,938 clients, and provided over 2.44 million meals.   • We're proud to honor the University of South Carolina School of Social Work as this week's winner of Transitions' Outstanding Program Support Award! 🏆 Their collaboration provides us with dedicated interns, enhancing both student learning and our client services.   • Midlands Gives 2024 was an incredible day of giving for Transitions Homeless Center. Thanks to the generosity of so many, we were able to raise a total of $43,373!   This amount will cover the cost of three beds at Transitions Homeless Center for an entire year. We are grateful to our donors and the 'Central Carolina Cheerful Giver' for helping us raise $21,500 for our matching funds. These contributions will directly impact the lives of individuals experiencing homelessness, providing them with a safe and supportive environment to rebuild their lives. I am proud of the work our staff are doing each day to end homelessness in the Midlands, one person at a time.    Thank you for your continued support of Transitions Homeless Center.
