We were founded by two therapists with a simple promise of providing exceptional skilled care, always with an emphasis on measurable outcomes for each patient. With so many home health providers to choose from, we stand out because of our ability to identify each patients' unique needs. We seek to understand each individuals' strengths and limitations so we can develop a comprehensive individualized care plan. INDIANA! WE ARE HIRING! 🔥 🔥 🔥 LUXE Home Health is looking for a Director of Nursing, RN for our Indiana Office. Please, email your resume at info@luxehh.com QUALIFICATIONS: Two years of experience in Home Health care, required Two years experience in a supervisory position, required Knowledge and ability to apply community health principles and practices. Knowledge of Agency policies and procedures. Ability to supervise, guide and develop skills and performance of service personnel. Ability to exercise independent judgment. Ability to work with individuals. RESPONSIBILITIES: Ensure compliance to Medicare regulations Service as a role model for all colleagues by setting an example of high standards in conduct, cooperation and job performance. Supervise the home health field staff including registered nurses, physical, occupational and speech therapists and certified nursing assistants Serves as a resource person to employees. Provides 24 hour/day, seven (7) days/week on-call coverage. Ensures that patients’ plans of care are developed, implemented and evaluated. Conducts monthly patient case conferences, in-services, staff meetings and maintains documentation; participates in community education projects. Ensures compliance with federal, state, ACHC, local and Agency policies in all patient care aspects of the Agency. Oversees staffing and visit assignments. Addresses and corrects all patient concerns.
