CMS is pleased to announce the launch of the Guiding an Improved Dementia Experience (GUIDE) Model – the first model of its kind to reach and provide support for people with #dementia and their #caregivers. Through the Guiding an Improved Dementia Experience (GUIDE) Model, nearly 400 participating organizations building Dementia Care Programs are serving hundreds of thousands of people nationwide: https://lnkd.in/eUNJ-B-w. Participating organizations represent a wide range of health care providers, including large academic medical centers, small group practices, community-based organizations, health systems, hospice agencies, telehealth organizations, and other practices.
