At A Healthier Democracy, we're addressing the growing health disparities in the U.S. Recent data shows a troubling increase in the mortality gap between rural and urban Americans. Matthew Roach, a vital statistics manager for the Arizona Department of Health Services, has tracked these disparities. His findings are stark: rural Americans aged 25 to 54 are dying from natural causes like chronic diseases and cancer at significantly higher rates than their urban counterparts. The gap has grown dramatically over the past two decades—from a 6% higher mortality rate in 1999 to 43% in 2019. Women in rural areas are particularly affected. Their natural-cause mortality rate increased by 18%, while their male peers saw a 3% rise. Cancer and heart disease are the leading causes, with lung disease and hepatitis also contributing. Pregnancy-related deaths are a significant factor as well. The situation is even worse for non-Hispanic American Indian and Alaska Native people. Their mortality rate increased by 46%, with Native women experiencing a 55% rise. The South shows the widest rural-urban mortality gap, exacerbated by many states' refusal to expand Medicaid. These disparities are a national crisis. The lack of healthcare access in rural areas is compromising the health of rural Americans and our democracy. Rural hospitals are closing, and essential health services are diminishing. This is compounded by higher rates of poverty, disability, and chronic disease, and fewer physicians per capita in rural areas. At A Healthier Democracy, we believe improving rural health requires a community-focused approach. It's not just about keeping hospitals open—it's about ensuring communities have the resources they need to thrive. This includes expanding Medicaid, increasing access to preventive care, and addressing social determinants of health. The COVID-19 pandemic has further complicated the public health landscape. We need comprehensive data to understand its full impact on rural-urban life expectancies and to guide our efforts in closing the mortality gap. We must advocate for policies that expand access to care, support healthcare infrastructure, and address the unique challenges faced by rural Americans. The health of our rural communities is the health of our nation. Together, we can build a healthier, more inclusive country.
