Discover how we're advancing population health management by optimizing care delivery in the home and community settings. This episode of our Healthy Days at Home podcast focuses on an integrated, whole-person approach to health, coverage and care. Hear from Executive Medical Director of Population and Curated Health Anil Singh MD, MPH, MMM; Senior Vice President of Population and Curated Health Enablement Chad Evans; and Vice President of Helion Nick Stupakis. Episode Highlights: 🔵 Nick, Chad and Dr. Singh share their personal experiences with caring for aging parents. As the population continues to age, virtual solutions and caregiver resources become increasingly important for managing health and care in the home. 🔵 Close collaboration and transparency with providers enables our blended health organization to understand clinician pain points, address them as an insurer and build innovative solutions to implement and test in our owned hospital network. 🔵 Population health is a key pillar of Highmark Health’s #LivingHealth strategy. To improve member outcomes, Dr. Singh and Chad emphasize that care must encompass all aspects of health: physical, social, mental and behavioral. 🔵 Together, these leaders are focused on integrating care, optimizing experiences and reducing duplication and fragmentation in healthcare. By implementing predictive models to anticipate patient needs, providers can connect patients to needed services right at the site of care. Kevin is a community health worker with ChristianaCare (one of our partners in Delaware) and is assigned to help dozens of patients with mental and behavioral health challenges. Before he can get those patients into clinical programs, he needs to first build trust with the entire family and understand the everyday challenges that make it harder for them to get care. We performed an analysis on 800,000 social determinants of health assessments of our own patients and members. Although there is some variation by community, the top five needs across our footprint are: financial resources; health literacy; housing instability; social connections and safety. The study, published in JAMA, highlights the need to include individual and local data when building plans to address social drivers of health. We believe health systems, employers and government agencies should expand individual SDoH assessments to better inform care, program design and investments.
