CMS' proposed 2025 Physician Fee Schedule includes a 2.8% reduction in payment rates, a new advanced primary care management bundle, and updates to payment and coding for cardiovascular risk assessment and care management, aiming to improve access to behavioral health services and maintain certain telehealth flexibilities. Additionally, the rule introduces six new MIPS Value Pathways and prepaid shared savings for successful ACOs. CMS has proposed a rule to increase Medicare payments for end-stage renal disease (ESRD) facilities in 2025. A rise of 2.2% in Medicare payments for ESRD facilities in 2025, with hospital-based facilities receiving a 3.9% boost and freestanding facilities a 2.1% increase are some of the proposed changes. The proposal expands ESRD outlier services, updates payment amounts, and introduces a two-tiered low-volume payment adjustment (LVPA) system. Oral-only renal dialysis drugs will be included in the ESRD PPS bundled payment system by 01 January 2025.
