About us

Health Services Advisory Group, Inc. (HSAG) is both a diversified Arizona-based quality innovation network-quality improvement organization (QIN-QIO) and an external quality review organization (EQRO). It was founded in 1979 by a group of medical professionals whose goal was to make a difference in health care quality improvement. As a QIO, HSAG works to improve the effectiveness, efficiency, economy, and quality of services delivered to Medicare beneficiaries. As an EQRO, HSAG conducts health plan-specific reviews of quality of care, timeliness, and access to services provided to beneficiaries in managed care, behavioral health, and prepaid inpatient health plans for Medicaid programs across the country. There is a QIN-QIO representing each state within a QIN region. HSAG is the QIN-QIO for Arizona and California. At its headquarters in Arizona, HSAG has three divisions: the Federal Division, responsible for Medicare work under a federal contract; the State & Corporate Services Division, responsible for Medicaid quality review work; and the Survey, Research & Analysis Division, which offers outcomes measurement, satisfaction surveys, and quality improvement interventions. It is HSAG’s mission to be a positive force in health care by providing helpful information and quality expertise to those who deliver and those who receive health services.

  • With two years of able guidance from HSAG, the second largest nursing home in the U.S. with 769 licensed beds—Laguna Honda Hospital & Rehabilitation Center—has regained its nursing home certification from Medi-Cal, the state Medicaid agency. Back in April 2022, Laguna Honda had been stripped of its certification by the Centers for Medicare & Medicaid Services (CMS) following four substandard surveys exposing failures in its staff training and competency, patient-centered care, and infection control activities. The San Francisco Department of Public Health reached out to HSAG to assist Laguna Honda as it prepared for recertification. With a focus on patient-centered care, leadership realignment, mentorship, and training, HSAG guided Laguna Honda to meet its action plan milestones.   • NET Program - Lunar New Year Celebration As part of a Centers for Medicare & Medicaid Services (CMS) nursing home effort, HSAG launched the Neighborhood Enrichment Team (NET) Program to help nursing home residents connect with their community and enrich their quality of life. Through the NET Program, HSAG partnered with Mission Palms Healthcare Center, in Westminster, California, and coordinated with Bolsa Grande High School's Vietnamese Student Association, to perform the Lion Dance and Fan Dance to celebrate the Lunar New Year for its residents.
