We’re excited to share that we’ve received a $2 million gift from the Yield Giving Open Call! The #YieldGivingOpenCall – managed by @Lever for Change – focused on elevating organizations working with people and in places experiencing the greatest need in the United States – and our organization does just that through effectively training community residents as peer educators to facilitate evidence-based chronic disease self-care and preventive education in their neighborhoods. Health People is grateful for the opportunity to expand the Diabetes Self-Management Program (DSMP), for example, in which people with type 2 diabetes to make lifestyle changes that lower their blood sugar and reduce the risk of complications or treatments, such as dementia, dialysis, and lower limb amputations. When Health People brought the program into city homeless shelters, there was a 45 percent drop in diabetes-related emergency room visits!
