The Care Coordinator is responsible for developing/facilitating educational and treatment groups, creating individualized service plans, and aftercare planning. The Care Coordinator provides assessments, establishes and maintains a counseling relationship with clients; assesses and analyzes pertinent information to develop individualized service plans that address client needs, program, and community concerns; maintains relevant information and submits reports as necessary. The Recovery Specialist facilitates psycho-educational and treatment groups, completes assessments, establishes and maintains a counseling relationship with clients, and analyzes pertinent information for purpose of developing discharge plans and addressing community concerns. 𝗔𝗽𝗽𝗹𝘆 𝗛𝗲𝗿𝗲----> https://lnkd.in/egk5VtED The Stonybrook Stabilization & Treatment Center is not your average substance use disorder treatment facility. Created by Sheriff Nick Cocchi in 2018 to help people battling addiction, the opening of the facility more than doubled the number of treatment beds in the western half of Massachusetts. Although the facility is operated under the umbrella of the Hampden County Sheriff's Office, it is a standalone facility with specially-trained treatment staff focusing exclusively on SUD and addiction-related issues. The Stonybrook Stabilization and Treatment Center provides evidence-guided, trauma-informed, and family-focused medically-managed withdrawal and post-withdrawal care in Ludlow, Massachusetts. Psycho-educational programs draw from proven models with Substance Use Disorder and Criminal Justice clients, scaled for appropriate engagement in the weeks following withdrawal to build a foundation to help someone successfully transition to long-term recovery.
