As health centers and communities respond to crises in maternal health outcomes, the Geiger Gibson Program in Community Health partnered with the Episcopal Health Foundation to explore how the Texas Medicaid managed care program addresses key aspects of maternity care. Building on a prior study undertaken with support of The Commonwealth Fund, the Geiger Gibson team conducted an extensive review of Texas' standard managed care contracts and contractual requirements related to maternity care. Following the contract review, a focus group conducted in collaboration with the Texas Association of Community Health Centers (TACHC) discussed challenges and prospects for strengthening maternity care and explored health center knowledge of and interest in new maternity payment approaches. With widening disparities affecting Texas' underserved communities - including low-income families and families of color - the Medicaid managed care has emerged as a powerful tool for improving maternal and infant health. The team's final report, Opportunities for Strengthening Maternal and Infant Health in Texas’ Medicaid Managed Care Environment, features the findings of the in-depth contract analyses and focus group, and highlights opportunities to improve community health center maternity care capacity and access. In a new blog on The Commonwealth Fund’s To the Point, the Geiger Gibson Program in Community Health examines the challenges experienced by community health centers following the Supreme Court’s 2023 decision In Dobbs v. Jackson Women’s Health Organization. With support of the Commonwealth Fund, our team conducted focus groups with staff from CHCs and state primary care associations in 15 states including 11 that have restricted abortion access and four adjacent states. We found a system under duress, with many providers reeling from the compounding effects of Dobbs, Medicaid unwinding, and resource, capacity and workforce constraints driven by local and national changes in the delivery of health care services. Our blog underscores the importance of community health centers in providing essential maternal and reproductive health care services, and the need for predictable funding and enhanced federal support to meet the increasing demand for a wider scope of high-quality maternal and reproductive health services in medically underserved areas.
