Our Complete Care at Severna Park PA-C, Chase Neitzke and Virginia Noftsier, Director of Operations, both Adfinitas Physicians, presented the Post Sepsis Clinical Pathway that was successfully piloted at the center to University of Maryland BWMCs SNF Sepsis Learning Collaborative. With the Adfinitas and Severna Park pilot, we saw a reduction of sepsis 30-day overall readmissions by 23% at Severna Park Center!
