Meet our Board Members! Mark Franciosa is the Chief Medical Officer for Community Health Connections! "Dr. Franciosa was named Community Health Connections’ Chief Medical Officer in March 2021. Prior to joining CHC, Dr. Franciosa worked at Sound Physicians for five years as the Regional Medical Director/Hospitalist and Population Health Medical Director where he developed and managed Population Health Programs at 28 separate hospitals from Maine to Georgia. Previous to that, Dr. Franciosa cared for patients as a Hospitalist and ACO Physician at Beth Israel Deaconess Hospital for six years. Dr. Franciosa trained in Internal Medicine at Boston Medical Center and participated in research projects in Vellore, India, Milagro, Ecuador, and Lusaka, Zambia.'' About us
Community Care Cooperative (C3) is a 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by Federally Qualified Health Centers (FQHCs) in Massachusetts. In 2016, leaders from 15 FQHCs throughout the state formed C3. An innovative model for an Accountable Care Organization (ACO), C3 leverages the proven best practices of ACOs throughout the country. C3 is the only ACO in the state founded and governed by FQHCs exclusively focused on advancing integrated and coordinated community-based care for MassHealth members. C3 is building its organization on the collective strengths of health centers. We currently have 17 partner health centers across the state and our continued growth and expansion enables C3 to better serve MassHealth members across the Commonwealth.

Two representatives from each of our MEMBER Federally Qualified Health Centers #FQHCs serve on our Board to guide and inform our work. Why does this matter? Each of our health centers is unique. And the skills, experiences, and “voices” brought to our Board by these individuals allows us to produce the medical programs and care plans to deliver great health to all of our members. Meet Damian Archer, Chief Executive Officer at Outer Cape Health Services! "Dr. Damian Archer is a board certified Family Medicine physician. He is a Bahamian national and previous Bahamas National Merit Scholar. He completed his undergraduate degree at The University of Western Ontario, London, Ont., Canada with a B.Sc. Chemistry (Dist.) in 2001 and was a successful MBBS (MD) and B. Med Sci (Hons) candidate at the University of the West Indies, Bahamas Clinical Campus in June of 2006. After completing his medical internship and Senior House Office duties with the Department of Anaesthesia at the Princess Margaret Hospital in Nassau, Bahamas, he transitioned to the St. Luke’s Family Practice Residency Program at Aurora St. Luke’s Hospital in Milwaukee, Wisconsin in July of 2008. He completed his residency as chief resident in July of 2011 and later that year started independent family practice with North Shore Community Health at the Peabody site in October of 2011. He currently serves as the Chief Executive Officer for Outer Cape Health Services. Dr. Archer’s wider interests in medicine include medical education and he collaborates with Tufts University School of Medicine and Boston University School of Medicine as a clinical instructor." Meet our Board Members! Marjorie Janvier is the Chief Medical Officer for Upham's Community Care. "Dr. Janvier received a Bachelor of Science in healthcare management and policy and a Bachelor of Applied Science in Bioengineering from the University of Pennsylvania. She obtained her Medical and Master of Public Health degrees from Tufts University School of Medicine and completed her Internal Medicine residency and chief residency at Caritas St. Elizabeth’s Hospital and an Infectious Diseases Fellowship at The Miriam Hospital/ Brown University. During her residency, Dr. Janvier discovered a passion for community health and commitment to providing HIV positive patients with quality, integrated health care. “The way that patients open up about their greatest hopes for themselves and their fears is very intimate. It is a great responsibility to understand these and to provide guidance.” In 2015, Dr. Janvier joined the Adult Medicine staff at The Dimock Center, where she provided primary and HIV specialty care for over 800 patients. During her eight years at Dimock, Dr. Janvier held several leadership positions, including Director of Care Management, Medical Residency Director and Clinical Director of Adult Medicine. Meet our Board Members! Say hello to C. Marie-Andrine Constant, Chief Population Health Officer at Outer Cape Health Services! "Dr. Marie Andrine Constant is a board-certified family physician, a public health practitioner, and public health advocate. Most recently, she was employed as a Supervising Physician at Mass. General Brigham Urgent Care in Lynn, MA. Previously, she served as the Associate Medical Director for the Lynn Community Health Center, working alongside an interdisciplinary team to provide patient-centered care and innovate care models for individuals with dual diagnoses of mental health and chronic disease. She earned her medical degree from the State University of Haiti, School of Medicine and Pharmacy, and her MPH from UMass Amherst, with a focus on diabetes and health disparities. In addition to English, Dr. Constant is fluent in Spanish, French, and Haitian Creole. As OCHS’ first Chief Population Health Officer, her role is to optimize care delivery, administer population health programs, support clinician performance and education, and achieve successful outcomes related to value-based care performance."
