Comagine Health is delighted to announce we have signed an affiliation agreement with Quality Health Associates of North Dakota (QHA) that provides a pathway to a potential merger of the two organizations. Through this new affiliation, Comagine Health and QHA will be able to become even more powerful drivers for health care improvement. Comagine Health has a long-standing history with the Centers for Medicare & Medicaid Services (CMS) serving as a Quality Innovation Network-Quality Improvement Organization (QIN-QIO). QHA is among multiple partner organizations joining Comagine Health in preparing to serve as the 13th Statement of Work QIN-QIO in Region 6, which covers 14 states from the Pacific Northwest through the Midwest, including North Dakota. We’ve teamed up with the Oregon Wellness Network (OWN) to launch a survey of Oregonians in order to understand their health concerns and perceptions of diabetes. Funded by the Centers for Disease Control and Prevention, the survey is open through July 31 and will help Comagine Health, OWN and our partners: • Capture the priority health concerns within the community • Understand the perceptions of Oregonians surrounding diabetes and its impact on their lives and the lives of their loved ones • Use survey results to inform program recruitment and delivery, and support funding applications •Reach as many community members as possible in the targeted counties For more information and to access the survey, visit https://lnkd.in/gKTUiybh. #Diabetes #CommunityHealth
