A new study from the Journal of the American College of Cardiology found that Black Americans experienced nearly 800,000 excess deaths from heart disease, and 1.6 million excess deaths overall, from 2000-2022. These numbers highlight the significant ways in which healthcare inequity and other social determinants of health directly impact care outcomes for the Black community. In order to see patient outcomes improve, we must find ways to deliver comprehensive, patient-centered care that takes these SDOH into account. Our team at Chamber is proud to support cardiologists in this effort, providing evidence-based insights, contracting support, and more so cardiologists can feel empowered to prioritize quality of care for every patient. Every year, almost 695,000 people die from heart disease in the U.S., equivalent to one out of every five deaths. And yet the American Heart Association estimates that 90% of these conditions could be prevented through proper education and action. At Chamber, we’re dedicated to preventing heart disease through value-based care that prioritizes quality over quantity. With this kind of model, underlying factors in heart disease, such as social determinants of health, are easier to address. Check out our guide to SDOH here: https://lnkd.in/eDVNiVgb

We know that social determinants of health like access to housing, education, income, literacy, discrimination, and more can all directly affect a person's risk of heart disease. So, why isn’t SDOH as widely addressed as it should be? In short, fee-for-service models in U.S. healthcare just don’t accommodate the quality of care necessary for a patient-centered approach. But Chamber has a solution. With our technology, cardiologists can make the switch to value-based care, pain-free. Learn more about how (and why) we do it at: https://lnkd.in/eDVNiVgb
