CMS has released the evaluation findings for the first five performance years of the Vermont All-Payer ACO Model: https://lnkd.in/eq83qWGD. This model aims to provide Vermonters with better medical care by tying payment to the quality of care given, rather than the number of tests, procedures, or office visits. This is also known as #ValueBasedCare: go.cms.gov/VBC. Providers emphasized the model’s value in providing a focal point for collaborating on care delivery transformation and population health initiatives. Overall, the model improved understanding and acceptance of value-based care among providers and inspired collaborative population health initiatives. States can now learn more about applying to the Cell and Gene Therapy (CGT) Access Model: https://go.cms.gov/3RMlkKc. The model aims to improve the lives of people with Medicaid living with rare and severe diseases by increasing their access to potentially life-changing treatments. New CMS #MaternalHealth opportunity! CMS just announced the Notice of Funding Opportunity for the Transforming Maternal Health (TMaH) Model. The goal of this model is to enhance care for mothers and infants enrolled in Medicaid and CHIP, focusing on improving health outcomes during pregnancy, childbirth, and postpartum.
