At Cadence, we have daily conversations around what we call “mission wins,” that is, examples from our patients, partner providers and health system partners that exemplify our mission of delivering life-changing treatment to 1 million people with chronic conditions so they can live healthier, longer lives. Earlier this week, one of our long-time enrolled chronic heart failure patients shared with our Care Team: "I'd be dead if it wasn't for this program!” At the time of their enrollment in Cadence, the patient could not walk to their mailbox. Now, they are responsible for taking care of their toddler grandchildren for the whole month of June. This involves trips to the pool, walking beside them as they ride bikes, and all the other joys of childhood summers. The patient is pretty impressed with themselves, as are we! For those tracking the remote patient monitoring reimbursement debate, Gabriel Perna's latest piece in Modern Healthcare is a must-read. A proposal to lower the monitoring threshold for reimbursement was postponed, highlighting the ongoing struggle to balance utilization and prevent fraud. Cadence’s founder and CEO Chris Altchek shared, “The AMA has to strike a balance to encourage the right kind of utilization. In a newly published manuscript in the Journal of Cardiac Failure, comprehensive remote patient monitoring and care improved clinical outcomes and reduced total cost of care in heart failure patients. Dr. David I. Feldman, cardiovascular medicine fellow at Massachusetts General Hospital and first author on the manuscript, shares of the results: "Our usual way of caring for heart failure patients isn't working. We need to consider all options when it comes to maximizing GDMT and helping patients with heart failure live longer, healthier lives. Collaboration with a centralized RPM program to optimize GDMT titration efforts is a necessary and impactful solution to help close heart failure care gaps and improve outcomes at scale." If you’re curious about the ins and outs of the American Medical Association’s CPT Editorial Panel’s upcoming review of the 15 codes used for remote monitoring of patients, Medical Economics gives an in-depth explanation around the potential for new billing codes that could give physicians more flexibility — and reimbursement — for RPM programs. Proposed code changes could lead to payment for more of the time physicians spend caring for their patients. In his congressional testimony last month, Cadence’s founder and CEO Chris Altchek shared that our experience with Medicare beneficiaries and PCPs “has made clear that RPM is key to the future of primary care.” Cadence’s customer base is 95% primary care physicians who want to improve how they manage patients’ chronic conditions outside of office visits. Dr. Feldman shares: “By combining technology and clinical practice guidelines in high quality RPM, we can make people live longer, spend less time in the hospital, and for those of us involved in paying for healthcare, save a whole lot of money. Did you know that Cadence patients are, on average, 250% more likely to trigger an alert when they transmit a vital reading between 11 p.m. and 3 a.m? Cadence’s Medical Director, Randall Curnow, MD, MBA, FACP, FACHE, FACPE, explains, "patients often are most vulnerable emotionally and physiologically in the late evening and early morning, times when they may need the most support but paradoxically have least access to care.” At Cadence, managing these overnight alerts has proven effective. In a new analysis, Cadence’s Robert Ritchie shares data showing that for every 10,000 patients we support, our NP-led Care Team has over 11,000 monthly encounters, only ~40 of which result in an escalation to the ED or 911. Ultimately, Cadence helps patients unlock greater personal independence, confidence, and quality of life. In a new piece in Healthcare IT News, Providence’s Chief of Virtual Care and Digital Health Eve Cunningham MD MBA and Family Practice Physician and Medical Director Dr. Kenneth Kooser discuss how RPM is improving patient health and reducing provider workload. Providence now has more than 1,500 patients currently enrolled in the care model with expectations for this number to reach 5,000 to 8,000 by the end of 2024. 88% of patients are taking their vitals daily and 77% remain enrolled in the program after six months. Dr. Cunningham shares of the partnership: "The program has since helped us keep chronically ill patients safe at home to achieve the Quintuple Aim of healthcare improvement: Improve the patient experience by tailoring treatments and interventions to patient data; address population health through proactive, preventative care delivery; lower cost by connecting patients to the most appropriate care intervention and reduce avoidable hospitalizations; improve provider well-being by extending their reach via a virtual care team; and provide equitable care by reaching all patients, regardless of their ability to present in-person for treatment." With primary care shortages accelerating nationwide, Cadence’s integrated remote patient monitoring program is proving to be a key to better healthcare experiences for providers and patients. Dr Matthew Else, a family physician at Ardent Health’s Hillcrest Healthcare System based in Oklahoma shares his experience in implementing RPM to improve care for chronic disease patients between clinic visits: “By keeping close track of patients between their visits, I am better able to manage their conditions and improve their overall health more quickly.” Dr. Else adds, “The program is equally beneficial to me as a practicing physician. Knowing experienced clinicians are monitoring patients’ blood pressure, blood sugar and weight — and catching small trends in the wrong direction — has taken weight off my shoulders and greatly reduced the pressure on clinic staff.” Now monitoring more than 1,400 patients across Utica Park Clinic, we look forward to growing the program this year to extend this primary care infrastructure across the region. Read Dr. Else’s piece in HealthTech Magazines here: https://lnkd.in/e6ShAfQp
