As the population of older adults grows, hospitals must adapt to meet their unique needs. The primary goal of providing age-friendly care is to follow four evidence-based elements of high-quality care for seniors, known as the 4Ms: managing Medications effectively, ensuring Mobility, addressing Mental health, and prioritizing what Matters most to patients. In this HealthLeaders article, Burke's CMO, Dr. Mooyeon Oh-Park, breaks down the 4Ms and shares how we use innovative tools like the CatchU app for fall risk and "Get to Know You Boards" for personalized care to help create a supportive, age-friendly environment. https://lnkd.in/eXJc7aXK
