  • We have come so far in HIV testing, status awareness, prevention, and treatment that an HIV diagnosis is not the same as it was 10 and 20 years ago. I strongly agree that every primary care provider needs to educate themselves on the innovative approaches that we know are very effective in reducing disease and death from HIV, and to offer these approaches to high-risk communities. One of these offerings is PrEP (pre-exposure prevention) for HIV and in addition to the daily pill Truvada and Descovy a new injection that can be administrered every 60 days for prep is aproved and available this year: Apretude. For appropriate patients, this new form of PrEp makes it even easier to reliably and effectively prevent HIV infection. Its very exciting to be a part of this new frontier in preventing the spread of a previously devastating infection.   • Its Black Maternal Health Week and Dr. Shirin Peters was privileged to have important conversations with the amazing founders of She Matters Inc on postpartum maternal health. The US rates of postpartum morbidity and mortality for black women are upwards of 4x non-black women. This astounding disparity CAN be solved with attention, access to postpartum primary care, education for pregnant women, and cultural competency anti-bias training for healthcare professionals and institutions. This company is working tirelessly to move the needle on closing this gap and I am honored to support them as a member of the Advisory Council. Access to culturally competent primary care in the 12 month postpartum period is very important for all women, and particularly for black women. Some of the most serious conditions that can happen in the postpartum period are: Postpartum hemorrhage. Postpartum preeclampsia. Deep vein thrombosis (DVT). Pulmonary embolism. Sepsis. Mental health conditions like postpartum rage or postpartum psychosis. Suicide Some symptoms that should prompt an urgent visit with a primary care provider or ER that you trust: Heavy vaginal bleeding. This generally means you’re filling a pad every hour for several hours. It can also mean that you’re bleeding more each day instead of bleeding less. Passing several large clots. A large clot is typically anything larger than a quarter. Passing one large clot is OK, but passing many could be a sign of a problem. Fever higher than 101 degrees Fahrenheit (38 degrees Celsius). New pains or an increase in pain. You should generally feel better over time, not worse. Foul-smelling vaginal discharge. Pain in your legs or ankles, especially
