About us

Alera Health creates integrated communities of care (ONEcare Networks) that meet people where they are and empower resilience and recovery. That reduces their need for crisis care, improves their lives and health, and frees-up resources for other patients. Alera Health specializes in building Integrated Systems of Care (ISOCs) designed to promote insight and communication between providers of primary, behavioral health, hospital, and social care services. Great health outcomes are not an accident. They are born of good communication. Our integrated ONEcare Networks bring behavioral health, primary care, hospitals, social care providers, and payors together to “Save Lives, One Population at a Time.” Our population health management services and technology improve outcomes for everyone by identifying disease early, prioritizing care, and building trust. Unlike most conventional CINs and ACOs, all parties in the ONEcare Networks we create are incentivized and aligned to create patient engagement and improve outcomes. ONEcare participants embrace hi-fidelity care models, close care gaps, anticipate crisis events, and provide transitional care support to mitigate clinical and social obstacles that inhibit treatment engagement.

    June was a whirlwind of making a difference in healthcare, and we couldn't have achieved these milestones without your invaluable contributions! Here's a recap of some of the amazing things we accomplished together: 💼 Conferences & Events: We rocked the AHIP 2024 Conference at booth #1600! Our CEO, Mike Rhoades, and Banner Health Plans CMO, Sandra Stein, delivered keynotes, and we connected with tons of innovators about the future of value-based care with ONEcare Networks. At the i2i Spring Conference, our session "The Data is Speaking, Are You Listening?" with Lizzy Reklau, Jose Castillo, and Chris Magryta, MD, focused on leveraging data to improve communication between patients and providers. Data-driven care FTW! We joined the discussions on integrated care at the OPEN MINDS Strategy & Innovation Institute, gleaning valuable insights on transforming strategies into solutions for complex populations. We were proud to participate with the Integral Health Network of Southern Arizona (IHNSA)! And of course, we hosted our Inaugural ONEcare Symposium at the OPEN MINDS Conference! It was a fantastic day of celebrating our collective impact, sharing best practices with fellow ONEcare rockstars, and learning from industry leaders like Monica E. Oss, CEO of OPEN MINDS. ⭐Special Recognition: We were honored to present the very first G.R.I.T Award at the ONEcare Symposium. This award, dedicated to the late Jill Lineberger, celebrates exceptional resilience and perseverance in improving health outcomes.
