The opium poppy plant, Papaver somniferum, is a naturally-derived source of morphine and codeine as well as the related alkaloids thebaine, noscapine, and papaverine. Poppy seeds may be found in many food products such as breads, muffins, cakes, bagels, seasonings, and salad dressings, as well as commercial or homemade teas. Typical baked products contain between 1 and 4 g of poppy seeds per serving, though the opiate content in foods can be highly variable depending on the preparation method. Poppy seeds are not the direct source of opiates found in poppy seed products; rather, contamination with other parts of the poppy plant during harvesting may contaminate the seed surface. In this month's clinical update, we will highlight poppy seeds and discuss unexpected toxicology results due to possible poppy seed ingestion. “Researchers found strong evidence of an association between cannabis use disorder and schizophrenia among men and women, though the association was much stronger among young men.” This is according to a news release from The National Institute on Drug Abuse (NIDA), found here: https://lnkd.in/gD2r5uuH. Managing individuals with substance use disorder (SUD) and mental illness can be complex, and clinicians may benefit from tools to monitor treatment adherence. Definitive testing for marijuana and prescribed psychiatric medications can confirm medication adherence and allow for quantitative measurement of THC levels in urine to monitor elimination and any indications of re-use. The presence of counterfeit substances containing polysubstances and adulterants continues to vary and grow over time. The general population attempting to use medications recreationally are at an all-time risk of a potentially fatal overdose due to the lethal options available in the illicit drug space. All individuals are at a greater risk of harm when substances are adulterated with multiple compounds. It is important to remember both illicit and prescription drugs when focusing on drug-related visits to the ED. According to the 2022 report of the Drug Abuse Warning Network (DAWN), prescription or other opioids were reported in 39.4% of opioid-related ED visits and oxycodone was the most common opioid reported (32.9%). Medication compliance is extremely important to monitor when prescribing opioids or any drug that has the potential for abuse/misuse. Nitazene analogs are novel psychoactive substances which are structurally different from fentanyl but attach to the same mu-opioid receptor. Recent studies have demonstrated that drugs within the nitazene class more often than not have greater potency than illicitly manufactured fentanyl, prescription opioids, and other designer opioids. In Aegis’ testing, increasing co-detection of fentanyl, fentanyl analogs, and nitazene analogs demonstrate that the drug supply is becoming more adulterated.
