What are some effective communication strategies for physicians during a crisis?

This will include: 1. Creating clear and efficient communication channels within your healthcare facility or organization that are specifically designated for crisis situations. Ensure that all healthcare staff know how to access and use these channels for timely updates and information sharing. 2. Ensuring that healthcare professionals, including physicians, receive training in crisis communication. This training should cover not only the technical aspects of communication but also the psychological and emotional aspects of dealing with a crisis

In my strategic healthcare planning, it's evident that the adage "forewarned is forearmed" holds true. When managing NCDs, we often deal with sudden spikes in cases or unforeseen complications. Being proactive, having a contingency plan, and being well-acquainted with protocols can significantly streamline crisis communication, ensuring everyone is on the same page.

In addressing conflicts and medical errors, we must have a systematic approach. Firstly, like in any good doctor-patient interaction, try to establish what are the ideas, concerns and expectations of the parties you’d be addressing. We all know that if this is not basis of that interaction, you’d be on a hiding for nothing. Secondly, have a framework of reference. Ask yourself what are they asking of you? Just an explanation (can do wonders for closure)? An apology? Blood - i.e. do they want to see somebody punished? Or is it money? Finally, can this be provided? If you feel nobody is at fault, don’t offer one of those non-apology apologies because an apology is being demanded! It’d feel like you’re insulting their intelligence!

Know your team. Make sure that the people on the team have the personality as well as the skills to be able to handle themselves and their position during a crisis. Not everyone works well during a crisis situation.

During my data analysis work, I've come across intricate epidemiological data. Translating this to actionable insights requires clear and jargon-free communication. Especially during crises, simplifying complex data is crucial. It ensures that both medical professionals and the general public can act on the information effectively.

The use of technology to connect is a beneficial tool to utilize during a crisis. Many doctor’s offices now offer virtual visits and teleconferences to communicate with their patients and peers. By leveraging technology, doctors can continue having effective conversations and ensure quality patient care during a crisis.

The majority of my one-on-one experience with physicians was in my role as an Implementation Consultant with new computer systems. In the ER, for example, there is a lot of pressure and physicians understandably get tense and frustrated when they feel the system is holding them back. I would ask them to let me help them walk through a task. I used a very calm and decisive voice to reassure them I was there and knew what I was doing.

My favorite tool in medical school is see one, teach one and do one. As a leader in my organization, I made sure to lead by example. The goal is to have another physician to display in this example empathy. This will help teach others by “seeing one” then having the physician take on this new principles and apply it with their patients (doing one). In order to fully understand a principle, you must teach those lessons of empathy to other physicians. I feel this is the key to teaching.

I also advise physicians to sit down when showing empathy and compassion. It’s sends the message, “I have time for you,” and it has been shown to increase the patients perception of the length of time you have spent with them.

While engaging with communities for NCD awareness, I've observed the immense impact of empathy. Especially in crises, an empathetic demeanor - a simple act of listening or a reassuring gesture - can instill confidence and calmness in distressed individuals.

Share stories and experiences: Engage others by sharing relevant stories and personal anecdotes. These stories can help build connections, lend credibility, and provide inspiration during difficult times.

This element of honesty, openness, and transparency in crisis is central to trust and teamwork. There will be knowns and unknowns with reality based constraints. Fear often fills information gaps. I think a potential way to approach this is to ask about the rumors and fears and be vulnerable in sharing some of our own fears. If we want to encourage and highlight the importance of honesty, transparency, and human-centered communication, I believe we can model that one example at a time and be able to lead with “we don’t know that answer yet,” and “here is why we think this is the right next step based on what we do know today. And then ask, “what do you think, how could we improve this, what are we missing, we need your help.”

We can assume that under normal circumstances the vast majority of patients have some level of anxiety when they visit a healthcare provider. Imagine adding to that anxiety with a hurricane, wildfires, or a pandemic. Remember that anxiety is the greatest barrier to effective communication, so it should always be our goal to reduce patients’ anxiety.

In my policy advocacy role, integrity is paramount. During crises, transparency isn't just ethical but also efficient. When physicians lay out the facts, even if they're challenging, it fosters trust. And trust, during trying times, can be the bedrock of effective crisis management.

Be transparent: Openly share information with patients, families, and colleagues. Provide updates on the situation, risks, and potential outcomes. Use simple and clear language: Avoid medical jargon and communicate in a way that everyone can understand. Break down complex information into bite-sized pieces and repeat important details if necessary.

Having collaborated with various stakeholders for NCD-focused health policies, I've realized the power of cohesive action. In crisis scenarios, fostering a collaborative spirit is pivotal. It's about sharing knowledge, acknowledging others' expertise, and navigating challenges collectively.

Collaborate and coordinate: Foster teamwork and collaboration among healthcare professionals. Encourage open communication, share information, and work together to provide the best possible care for patients. Adapt communication style: Recognize that each individual may have a unique communication style, and adapt your approach accordingly. Some may prefer written information, while others may benefit from face-to-face conversations.

Through my work in strengthening healthcare systems, I've seen medical setups facing setbacks, be it from funding issues or sudden NCD outbreaks. Resilience isn't just about bouncing back but also evolving from each challenge. As physicians, adapting, learning, and moving forward with renewed vigor post-crisis can be our most significant contribution to the medical community.

I recommend having a routine or ritual that you always follow. For me, I use the SPIKES strategy: S – Setting P – Perception/Perspective I – Invitation K – Knowledge E – Empathy/Emotion S – Summary/Strategy

The key is to teach emotional intelligence in medical school. We learn ethics but we need to add this key important concept that we must truly do well. This will help the profession in communication.

I recall an instance during a sudden NCD outbreak in a regional community. Dr. Patel, a physician I had collaborated with, faced significant challenges in managing the crisis. But what stood out was his commitment to open communication. He held regular community sessions, addressing concerns, clarifying doubts, and always being transparent about the situation. His approach not only alleviated panic but also fortified the community's trust in the medical fraternity. It was a vivid testament to the power of effective communication during crises.

After the crisis subsides, conduct a debriefing session with your healthcare team to assess the communication strategies used and identify areas for improvement. Document lessons learned to enhance future crisis response.

Sometimes I find during a crisis or situation - instead of jumping in and trying to do everything it is best to listen and keep an open mind, analyze and data gather. Not every problem needs a solution immediately even though it may appear that it does. Often with this methodology, the answer usually presents itself within a high functioning team.
