Continuous improvement is very important in this point, and you have to ensure it is systematic and SMART (specific, Measurable, Achievable l, Realistic, Time bounded), this starts with HSE objectives and targets, and extends to review all HSE documentation in a regular period (for example annually) , this guarantees that you don't forget to update your procedures and that you review all your documents to be complying with recent legalisations, new projects, new activities, new technologies..etc
