3 What are the challenges of molecular imaging in cancer detection?

Molecular imaging has the potential to detect early-stage cancer with high sensitivity and specificity, however, there are some challenges that limit its ability. The availability and selectivity of molecular imaging agents can be difficult to design and synthesize due to the lack of well-defined or unique targets or receptors. Additionally, some agents may have low stability, toxicity, or immunogenicity. The resolution and contrast of molecular imaging techniques also pose a challenge as they can either provide high-resolution images with low contrast, or low-resolution images with high contrast. Optical imaging has high resolution but low contrast, while PET has low resolution but high contrast. Furthermore, signal attenuation and background noise can reduce the quality and accuracy of the images. Lastly, the complexity and variability of cancer biology makes it difficult to identify and image reliable biomarkers that reflect the true state and stage of cancer.

Bioengineers are constantly developing and improving molecular imaging agents and techniques to overcome limitations and enhance performance in cancer detection. Strategies and innovations include developing multifunctional and multimodal agents that can target, image, and deliver drugs; enhancing resolution and contrast of imaging techniques with advanced hardware, software, and algorithms; and exploring new biomarkers to reflect molecular diversity and dynamics of cancer. These approaches can provide more comprehensive information about the molecular and cellular status of cancer.

I would add that theranostic agents incorporate molecular imaging and therapeutics to both image and treat cancer at the same time. Multimodality imaging is very useful in preclinical research and the clinic.
