The first step is to schedule an intake and consultation appointment.
To do this, please call our office at: (309) 829-8703 on weekdays from 10a.m. to 5p.m.
If our phone lines are busy, please leave a message with a name, phone number, reason for calling, and the name of the city that you are living in.
Please allow 2 – 3 weeks for processing to book an appointment, our intake team will be in contact with you as soon as possible.

Please Note: Some intake appointments cost $25, to be paid upon arrival.
