RIGHT TO REPORT OR FILE A COMPLAINT
 
If you believe that your rights, or a CCP Policy, have been violated or if you are dissatisfied with our policies or procedures, you may file a complaint either with us, or with the federal government. CCP, as well as our affiliates, staff and partners are prohibited from retaliating against you or your family, or change our treatment of you in any way if you file a complaint.
 

OPEN DOOR POLICY

At any point that you would like to file a complaint with Transcend Charlotte, Inc., you may bring your complaint directly to your provider, their supervisor or the Executive Director.
 

CONFIDENTIALLY OR ANONYMOUS REPORTING

Confidentially and/or anonymously report a grievance or concern through Transcend’s secure third-party ethics reporting system. Contact the system via:
