A, A biopsy from the upper portion of the left arm showed well-formed granulomas composed of epithelioid histiocytes and a sparse lymphocytic infiltrate. Stains for acid-fast bacilli and fungal organisms were negative. Polarization was negative. B, Clinical image at baseline (while the patient was taking methotrexate 15 mg weekly).
