In addition to the cortical focus theory, based on studies in the WAG/Rij rat published by Radboud University Nijmegen (Meeren et al., 2002), we further unraveled the dynamic changes of the involvement of cortex and thalamus during generalized Spike-and-Wave discharges (SWDs) for human absence epilepsy using MEG. Gupta et al. (2011) showed that a cortical driving source initiates a cascade of events leading, as shown in the figure, to a repetitive pattern of bilateral synchronous generalized activity during the waves and bilateral focal frontal and parietal areas, with cortical involvement (Westmijse et al., 2009), during the spikes (Ossenblok et al., 2019). In a recent paper https://lnkd.in/gtRBTf24 the knowledge acquired in these studies was translated into ambulatory methods. Automated detection software was used to detect SWDs occurring in around-the-ears EEG arrays (cEEGrids) to capture 3-Hz short-lasting and ictal SWDs showing a similar course of bihemispheric synchrony, however, with a ‘dip’ preictally of the ictal versus the short-lasting SWDs. #Absences #SWDs #Ambulatory #cEEGrids #Automated #Detection #Bihemispheric #Synchrony

Really proud of my recent paper in Neurology discussing the ethics of diagnosing prodromal Parkinson's disease. As the era of disease modifying treatments for neurodegenerative diseases including #Parkinsonsdisease draws ever closer, we need to grapple with challenging concepts: risk, (un)certainty of progression, long term care, healthcare costs (and opportunity costs), not just as #neurologists, the medical profession in general, but also as a society, including law makers and above all the public. 🔥 🚀 Read our new paper published in the European Journal of Internal Medicine, investigating the effect of Glucagon-like peptide-1 receptor agonists and sodium-glucose cotransporter-2 inhibitors combination therapy versus monotherapy on major adverse cardiovascular events: https://lnkd.in/dBiUyyCe 💊 Our results indicate that combination treatment with GLP-1 receptor agonist and SGLT-2 inhibitor is significantly more effective in decreasing the risk for MACE, all-cause mortality and cardiovascular mortality, compared to either GLP-1 receptor agonist or SGLT-2 inhibitor monotherapy. 🎯 In individuals with type 2 diabetes and established CVD or at high risk for CVD, combination treatment with a GLP-1 receptor agonist and an SGLT-2 inhibitor should be preferred over monotherapy. Want to know exactly how many hours a day you should spend standing, sitting, exercising and sleeping? New research conducted by Swinburne University of Technology and the Baker Heart and Diabetes Institute has the answer. Apparently, the ideal balance that will give you the greatest chance of staying healthy is 6 hrs of sitting, 5 hrs & 10 mins of standing, 8 hrs & 20 mins of sleeping, 2hrs & 10 mins of light-intensity physical activity (such as making dinner) and 2hrs & 10 mins of moderate-to-vigorous intensity physical activity (such as a gym workout). Exciting news! 🎆 We are happy to share the publication of the latest study by our colleague Eva Kimpe in Frontiers in Oncology! This model-based health economic evaluation examines the impact of late radiation-induced small bowel toxicity in rectal cancer survivors by comparing different radiotherapy techniques. 📈 The study highlights the societal cost of small bowel toxicity in this population 💶. With the use of health economic modeling techniques, Three-Dimensional Conformal Radiation Therapy (3D-CRT), Intensity Modulated Radiation Therapy (IMRT), and Intensity Modulated Radiation Therapy - Image Guided Radiation Therapy (IMRT/IGRT) were compared. 💡 The findings of the study reveal that advanced techniques, such as IMRT and IMRT/IGRT, substantially reduce the occurrence of small bowel toxicity events. This underscores the importance of considering late toxicity effects in health economic evaluations and decisions related to investments and reimbursement, demonstrating the potential for long-term cost savings and improved quality of life through the adoption of novel radiation techniques. In order to improve the monitoring of a patient's response to therapy, a method is provided to set a plurality of allele frequency thresholds to accounting for variations among tumours and patients. Acute respiratory infections can trigger acute myocardial infarction. Annemarijn R. de Boer, MD, PhD, et al. aimed to quantify the association between laboratory-confirmed influenza infection and acute myocardial infarction, particularly in patients with and without known coronary artery disease.      This observational, registry-based, self-controlled case series study evaluated the association between laboratory-confirmed influenza infection and occurrence of acute myocardial infarction. Laboratory records on respiratory virus polymerase chain reaction (PCR) testing from 16 laboratories across the Netherlands were linked to national mortality, hospitalization, medication, and administrative registries. Influenza infection was defined as a positive PCR test result. Acute myocardial infarction was defined as a registered diagnostic code for either acute myocardial infarction hospitalization or death. Very glad for our new publication! A study conducted among hospitalized patients with COVID-19 pneumonia in our Department during 2020-2022 (n=990). We report the prevalence of long-COVID symptoms, the results of pulmonary function tests and CT findings and the clinical outcome at 12 and 24 months after acute phase of COVID-19. We also discuss the impact of Long-COVID on daily routine (days of absence from work, outpatient visits, hospitalization) and the differences among patients based on antiviral treatment, vaccination status and variants of concern. Next Monday (1 July), Martin Martinek will defend the thesis ‘Contemporary atrial fibrillation management: healthcare expenditures, treatment methods, and outcomes’ at 13.00h @ Maastricht University. ➡️ All publications of this PhD project have provided evidence that healthcare expenditures for ablation therapy are a reasonable investment in our AF patients. By using more effective and efficient but safe RFCA therapies, further improvements are possible. In persistent AF the adaptation to new treatment targets could improve outcomes and reduce repeat ablations. First-line PVI can clearly provide benefits over AAD and should be offered to our patients.
