CARE MANAGEMENT
Once a New Earth student is enrolled with New Earth, they are assigned a Care Manager who will examine their needs through a compassionate clinical assessment. Their Care Manager will help them create short term and long-term goals, ensure social-emotional health is treated through therapy, connect with family to close any gaps in care and to assess if any additional resources such as groceries or rental assistance is needed, and help discover areas of interest to start exploring during their time on campus.
WHAT A CARE MANAGER DOES FOR THE YOUTH
- Assess needs and goals of New Earth member with compassion, patience, and accuracy
- Coordinate daily check ins to monitor mental, emotional, and physical health
Assist New Earth member in exploring enrichment programming - Create actionable short-term and long-term goal plans
- Assist in navigating systems in order to obtain the necessary documentation and information needed for achieving a goal
- Support in vocational exploration by providing resources and searching for avenues for hands-on experience in different industries
- Connect with family to close any gaps in care and to assess if any additional resources such as groceries or rental assistance is needed
