Zusammenfassung
Die Synkope – ein kurz dauernder, spontan reversibler Anfall von Bewusstlosigkeit – stellt auch im Kindes- und Jugendalter ein häufiges Krankheitsbild dar. In der präklinischen diagnostischen Abklärung steht die Unterscheidung zwischen harmloseren, neural vermittelten Ohnmachtsanfällen sowie Synkopen aufgrund organischer Erkrankungen im Mittelpunkt. Mit Hilfe einer exakten Anamnese, körperlichen Untersuchung sowie einem Echokardiogramm ist es in vielen Fällen möglich, die Genese näher einzuordnen. Bei Verdacht auf eine organisch bedingte Synkope sind weitergehende Untersuchungen indiziert sowie eine adäquate Behandlung des Grundleidens einzuleiten. Unter den organisch bedingten Synkopen sind vor allem die kardialen Synkopen wegen ihrer hohen Mortalität hervorzuheben. Die neural vermittelten Synkopen sind in der Regel harmlos und durch einen exzellenten Langzeitverlauf gekennzeichnet. Im Folgenden soll das Krankheitsbild der Synkope in Kindes- und Jugendalter dargestellt werden. Besonderes Augenmerk gilt hierbei der Entscheidungshilfe für das praktische Vorgehen bezüglich Diagnostik und Therapie.
Abstract
Syncope represents a frequent complaint in childhood and adolescence which is characterized by a spontaneous reversible loss of consciousness of short duration. The key point in the diagnostic evaluation is the differentiation between harmless neurally mediated syncope and syncope due to organic diseases. An exact medical history, physical examination and a resting echocardiograph frequently provide enough evidence to clarify this. Further diagnostic procedures are required whenever organic diseases are suspected (neurologic, cardiac, psychiatric, endocrinologic/metabolic syncope). When syncope due to organic diseases is suspected specific therapeutic strategies for treatment of the underlying disease are necessary. An excellent prognosis is reported in neurally mediated syncope, whereas among syncope due to organic diseases, cardiac syncope in particular shows a high mortality. The aim of this article is to focus on the evaluation of pediatric patients with syncope in terms of diagnostic procedures and treatment options.
