Abstract
Primary ovarian insufficiency (POI) is defined as the development of primary (hypergonadotropic) hypogonadism before the age of 40 years. The diagnostic criteria of POI are irregular menses in women before the age of 40 years in association with follicle-stimulating hormone level in the postmenopausal range. Basic evaluation of women with POI includes a karyotype analysis, testing for an FMR1 premutation and for adrenal autoantibodies. Women with spontaneous POI should be screened for thyroid-peroxidase autoantibodies. A pelvic ultrasonography and measurement of bone mineral density are also indicated at the time of diagnosis. In women with POI, systemic hormone therapy should be applied until the average age of natural menopause. If the patient desires pregnancy, in vitro fertilization with donor oocytes or donor embryos is the most appropriate treatment.
