The Bridger/Peer Specialist is responsible for providing self-help and empowerment tools to participants in the community through participant led groups as well as providing individual services.
Essential Functions:
Model and promote the principles of recovery/self-help
Manage a caseload within program standards
Input information for 2010E application into PACT system and referral sources
Ability to navigate program systems. (AWARDS)
Develop a person-centered Individual Recovery Plan with the participants and their chosen support team
Assist clients with entitlement and provide linkage, and/or referrals to such services (SNAP), Medicare, Medicaid, SSI, SSD, reduced fare MetroCard and HRA, etc., Conduct wellness activities, Comply with all program regulations, experience working in a crisis management model, and additional duties as required
