As an Advanced Practice Clinician(APC) at knownwell, you will work alongside a team of committed physicians, APC’s, registered nurses, registered dietitians, and behavioral health clinicians to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health.

Schedule: Monday- Friday with flexible scheduling, 32 patient contact hours minimum. Hybrid model of care.
EMR: Athena
On-Call: Rotation 

Requirements:

    • Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB)) as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by the Physician Assistant certifying body (NCCPA).
    • Unrestricted DEA licensure and prescribe authority (per state practice regulation).
    • BLS certification
    • At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred
    • You have an active, unrestricted license to practice in Minnesota, or willingness to obtain.  Willingness to obtain additional licensures if needed based upon business needs.
    • You have excellent communication and interpersonal skills.
    • You’re dedicated to staying current with the latest developments in primary care and obesity medicine.
    • You believe in inclusive, compassionate care.
    • You believe in shared decision making with a patient. You honor the goals a patient has for their health, and you partner with them to help them achieve those goals. 
    • You have high emotional intelligence and know how to make a patient feel at ease – without judgment and without shame.
    • You’re willing and excited to try new technologies that are both patient and provider oriented.
    • You are excited to work with a collaborative team of physicians, APC’s, registered dietitians,  medical assistants, health coaches, and other staff.
    • We are unable to sponsor visas currently, so you are authorized to work in the United States.

Responsibilities:

    • Practice exceptional medicine within knownwell
    • Sees adults for primary care, often with a focus on metabolic health, or adults for metabolic health only.
    • Embodies the knownwell mission and values of patient inclusivity in how you deliver care.
    • Coordinates with the broader care team to ensure the patient has the support they need.
    • Documents as necessary to ensure accurate billing, including review of remote patient monitoring data.
    • Communicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician response.
    • See adolescents ages 12+ for metabolic health

    • Ensure patients feel known well
    • Ensure panel is open to any individual, regardless of their current health status and comorbidities.
    • Listen to the aspirations of the patient and ensure care is tailored to the patient’s unique health journey.
    • Provide an environment in which patients feel comfortable and free from judgment.
    • Offer exceptional encouragement as patients reach their goals.

    • Coordinate across the collaborative care team
    • Escalate any concerns to your dedicated site leadership team.
    • Provide clinical guidance to medical assistants and nursing teams as assigned.

    • Stay current with licensure and appropriate re-certifications.
